Admission

You should not forget anything, it is an important moment.
When you are hospitalized, you should bring the following documents :
  • identity card, passport or family record book
  • valid social insurance card
  • chart of mutual insurance company or private insurance
  • vaccination and health record for children
  • results of your last medical examination (radios, analyses...)
In certain cases, you will also have to bring :
  1. the section of the declaration of industrial accident (if you are victim of an industrial accident)
  2. the notebook of free care ( if you are an ex-serviceman who is a war victim)
  3. form E111 or E112 (if you are amenable to the European Union)
  4. the notification of any specific allowance you are entitled to (special education, handicapped adult allowance...)
